We are only as safe as me! It’s important VM employees feel empowered to speak up when they see safety concerns or problems in the workplace. This website and secure phone line provide avenues for VM employees to confidentially report issues so improvements can be made for a safer work environment. See the instructions below for filling out a report, or you can call (833) 232-SAFE (7233).

  1. Name. Check the box ‘I want to report anonymously,’ if you do not wish to be identified.
  2. Check the box if you would like to have a Safety team member follow up. Fill out the boxes providing additional contact information for the follow-up.
  3. Select mode of transportation and fill in concern and location. These are the only required fields.
  4. Share immediate actions taken and solutions/suggestions. This will help with next steps for the safety team.
  5. Upload image. You can choose to upload an image that is relevant to this report.
  6. Click Submit to file the report.
